Job Description:


The Executive Chef will lead the culinary efforts of the department and University through personnel education, training, product development, and audit.

As the culinary expert the incumbent will provide leadership and guidance to reach operating standards of excellence for all food service locations. While the bulk of the work will take place in the Residence dining the Executive Chef must be available to work other parts of our business.

Other duties include:

The Executive Chef will report to the Food Service Director and have 15+ associates under his/her supervision.The position will ensure the unit has strict compliance of the Compass Quality Assurance and Health and Safety Program, Occupational Health and Safety Act and WHMIS regulations. You will ensure Hazard Analysis and Critical Control Point (HACCP) standards are followed in the food preparation process.
Skills and Requirements

* Post secondary education plus a minimum of 3 years related management experience in a fast paced food service environment
* Red Seal Chef Certificate or equivalent is preferred
* Sound knowledge of financial management including ability to control food and labour costs
* Creative ability to apply current trendy food items and presentations to food service operations
* Strong supervisory skills, capable of motivating, leading and developing associates

Doorways to Care (DWTC) Outreach & Access Coordinator

• Finding the right Long-Term Care Home when living independently is no longer possible.



The Doorways to Care Network, is a collaborative initiative funded by the Central Local Health Integrated Network (LHIN) to enhance the quality and co-ordination of referrals for seniors and caregivers seeking community services. Through the use of a toll-free line, common referral processes, the use of a common referral tool, and supportive inter-agency phone transfers, elderly clients are provided with a supported navigation and access system. Now that the Doorways model has been developed and implemented in 22 agencies, the next phase of its development is aimed at increasing the volume of referrals, and also the public and professionals understanding about how Doorways to Care can help seniors who “don’t know where to start” when trying to find services.



PURPOSE AND KEY FUNCTIONS OF THE ROLE

1. Provide Community Service Linkage and Follow-through for Frail Seniors Leaving ER/ALC beds

• Establish processes to link clients to a range of community support services

• Develop processes to facilitate and encourage collaboration between community support agencies and GEM nurses, ER nurses and Hospital Based CCAC Case Managers following an elderly client’s risk assessment

• Develop processes to carry out “warm” referrals to appropriate agencies

• Establish mechanisms in hospitals to promote knowledge of DWTC and promote referrals to partner agencies

• Collect data to evaluate successes and opportunities for improvement



2. Community and cultural outreach to seniors and caregivers in the Central LHIN Key Audience – Seniors/Caregivers/Community Groups/Cultural Groups and General Public

• Lead the organization of events and / or partner with DWTC agencies to deliver outreach and education to seniors and caregivers.

• Initiate partnerships with DWTC agencies with an ethno-specific focus to deliver outreach and education to these populations within the central LHIN area.
• Research new contacts that can help in spreading the word about DWTC. For example, seniors groups, community groups, health and community support organizations, health and community programs in the Central LHIN, etc.
• Educate seniors, caregivers and the general public about DWTC by attending health care fairs, seniors and community events, etc.
• Carry out marketing and partnership outreach plans (e.g. distribute marketing materials,
• Research and book free advertising and editorial opportunities in local paper and provide required information/articles/advertising etc.

3. Health professionals outreach in the Central LHIN Key Audience – Health Care Professionals, Healthcare Provider organizations, etc.
• Develop educational modules to provide outreach to professionals and community providers
• Organize activities in partnership with DWTC organizations, to deliver education to health care providers that can help seniors/caregivers access community services.
• Partner with other Aging at Home programs, for example, Balance of Care, Home At Last, Medication Management Support Services, Family Caregiver Connections, and other programs in the Central LHIN, to promote programs to common audiences.

4. Support to Participating DWTC Agencies Key Audience – DWTC Agency Point Persons, Intake Persons, Leadership Staff, Steering Committee, and the Central Local Health Integration Network
• With Steering Committee support at agency leadership level, work with agency point persons and work groups to support the continued operations, maintenance and improvements to the DWTC model, including marketing and outreach activities within the DWTC network agencies.
• Conduct follow-up phone calls to clients regarding their level of satisfaction with the DWTC service.
• Create progress and status reports and present to Steering Committee on a monthly basis.
• Support the collection, analysis and reporting of evaluation data to Steering Committee EXPERIENCE/QUALIFICATIONS
• Degree in Social Work or related Health Care Discipline or equivalent, combined with a minimum of 3 years experience in a community-based setting, working with seniors and caregivers.
• Demonstrated ability and achievement in project co-ordination, delivery of educational information, data collection and evaluation.
• Excellent interpersonal, organizational, problem solving and communication skills (including the development of presentations)
• Knowledge and experience within the Central LHIN area, and the various Aging at Home, Emergency Room/Alternate Level of Care initiatives an asset.
